I cheaped out. I bought 3 peices of white Dal-Tile from Home Depot and glued them together. I use the suction cup to hold the camera to the tile, set it in the the opening on top of the dash.

I use this mounting bracket made by a small company that specializes in a few F-150 products. I really like it, and I get a lot of comments about it. You can use the base to attach a few things. If you look at the product pictures there is one with a phone mounted on a ram arm, and that is what i am using for my phone, i got a ram arm like the one linked below and a ram magnetic circie base, the magnetic connects to a small metal plate on my phone case, easy on and off, you can attach the metal plates to any sort of devise. i like that it brings the phone closer to my face/hands rather than being all the way out on the dash.
