Is there anything I can do to prevent the slide from getting out of adjustment again

Honestly, that would depend on what caused the issue the first time. It's possible they determined what the problem was and fixed it permanently.

So new to forums. My question is what can I do about the axle. Will have to replace it or can it be aligned.

I would replace it. Axles are cheap, and it isn't hard to do.
