Definitely check the bracket bolts every bearing service. Doesn't matter if you have discs from the factory or an aftermarket setup. The thermal cycling can cause dimensional change of the cast bracket vs. the fabricated flange causing originally torqued mounting bolts to loosen.

Do you think there should have been thread lock applied and someone forgot to do that?

Are these 7 K axles? This happened to me twice with Morryde IS suspension & disc brakes. The first time Morryde thought it was a fluke. The second time their engineers discovered that the bolts being used to secure the calipers to the bracket were inadequate. They replaced them with heavier bolts & nuts. Their engineers think the actual issue is the nuts but they upgraded both. So far everything is ok. Morryde paid for everything both times. You are lucky the calipers only came off. Mine rotated so far that the brake line sheared & I had no trailer brakes. My set up was Kodiak brakes but maybe Dexter was using the same bolts. I recommend you ask Dexter to replace the bolts on all 4 sets of brakes. This is what Morryde did.

They use a split lock washer, and one would think that properly torqued, it would not loosen. They do not use any thread lock.

Like what was said above, at least check them every year. I found at least two on each hub that were "tight", but only about half the proper torque amount. Per Dexter, that should be 75 ft lbs.

This is on the factory 8K axle/disc brake upgrade. Dexter never sent me the replacement bolt's, and I replaced them with ones that I got from a local commercial trailer repair shop. The nut is flanged and has a decreasing inside radius (don't know the proper name). Don't think they will come off, but all bolts will be checked several times a year now.

The only reason mine did not do what yours did, was the top plate on top of the spring pack for the axle u-bolts stopped it. Lots of metal missing on it. Glad you came out okay, the thought of losing the brakes with 15K lbs behind me is a bit scary.

Bleed the rear two brakes (one being the new caliper), but was unable to do the front two as the factory actually partially rounded the bleed screws when they tightened them. Need to buy some new ones before I force them off.

Trailer seems to work like normal, took it around the block. Dexter still hasn't said anything about the rim.
Did a trip several weeks ago, about 1,200 miles total, and everything worked, no bolts loosening.

After an exchange of emails, Dexter stepped up to the plate and covered the cost of the new rim (with tire).

It's a pain that you had to go through the trouble, but reassuring to hear Dexter made it right.
