Could be either one. Both my manual slide cranks and my spare tire crank look like that.

Is this sticking out toward the passenger side of the camper and does it look like it aligns with the center of the spare tire?
If yes, then it is your spare tire tensioner. It raises and lowers the spare tire to the ground. There is a metal plate attached to a cable that goes through the center of the spare tire, this is the pulley that you attach the long hand crank to which lowers the spare.David and Peggy

Its the manual crank for the slide out. If you have a slide on both sides ? there will be one on each side. the crank controls the slide on the opposing side. GL
