B&W Companion pivot arms location and height

I will be assembling my new hitch today. I'll be towing a 397 with a F450 long box. Seems like the consensus on the internet is to place them forward closest to the truck cab. I don't have the trailer here so hoping someone can chime in. Also, what height or which bolt holes for clearance? My truck is stock (wheels and suspension).

I allways ran my B&W with the arms in the middle but the rv had an extended pinbox. If you have the standard length pin box if you put the arms towards cab you probably will have a clearance problem with the tailgate/bumper

I have a Gen-Y executive torsion flex pin box.
