I have a disease that needs management also. I see a specialist now and then, and I usually have to book way in advance because it's often a new doctor and they schedule new patients farther out. That requires coordinating with our trip planning of course.

Every doctor I see, I ask that they give me their visit notes that I can bring to the next doctor, wherever and whenever that may be. That way there is at least *some* continuity of care even if it's often a different doctor.
