Momentum survey

I received a survey for a Momentum we purchased. The letter says the survey can be completed on line at gdrv.aimbase.com/csisurvey . This does not work. Anyone know how to get to this survey on line? Do to very bad hand writing I would rather do the survey on line. Thanks
